A doctor’s note refers to an official document that verifies the justification for your absence from work or school due to a common illness like a mild cold. If your employer requires proof of your sickness-related absence, they may ask you to provide a doctor’s note to confirm your reasons for staying home. Our Doctor’s note will state you had a valid medical reason for missing work or school.
The COVID-19 note is a note that specifically states COVID-19 as the reason for you missing work or school.
A Travel Note validates a medical reason for you missing or cancelling a flight.
Our regular and COVID-19 doctor’s notes are $39.99. Our travel notes, ESA (Emotional Support Animal) letters, and Customized notes are now $89.00 which includes a 15 minute video visit with one of our Board Certified Physicians and any additional forms that need to be completed for travel insurance claims.
Our service is available in all 50 states.
You can choose a maximum of 5 days per 30 day period on the doctor’s note. For a regular doctor’s note, you may select from a range of 2 days prior to and 5 days after the date of submission.
You may select dates from 5 days prior to and up to 5 days after the date of submission.
Your note will be provided to you in our secure portal.
