A doctor’s note refers to an authenticated document that certifies your absence from work or school due to a common illness, like a mild cold, by a licensed physician. If your employer requires proof of your sickness-related absence, they may ask you to provide a doctor’s note to confirm your reasons for staying home. Our Doctor’s excuse note states you had a valid medical reason for missing work or for missing school.

For a regular doctor’s note and COVID-19 notes, the cost is $39.99. For travel notes, the cost is now $79.99, which includes any additional forms that need to be completed for travel insurance claims. This increased cost of a travel note is reflective of the higher liability related to our notes being the basis for a travel insurance claim.

Our notes are accepted by employers and school across the nation. We stand behind and validate our notes should your employer or school call us for verification of the note. Our doctor’s notes are an official and legal document, medically excusing you from your absence due to a mild illness that required you to stay home and recover. Telemedicine is a valid and accepted form of healthcare in this country. 

A COVID-19 note is a doctor’s excuse note that specifically states COVID-19 as the reason for you missing work or school and explains the need for your isolation.

A Travel note validates a medical reason for missing or canceling a flight.

Your note will contain the letterhead of our medical group, HEALTHSOURCE Medical Associates. It will not include mydrsnote.com on the note. It will also state your name, the date(s) of your absence, and that you had a valid medical reason to stay home. Your notes will be signed by a Board-certified physician licensed in your state.  There will also be a QR code which can be scanned to show the credentials of the doctor. If you need more details added to your note, you will be able to request for custom changes.

Our service is available in all 50 states. 

You will be able to select a maximum of 5 days per 30 day period for your excuse note. You may select days from 2 days prior to and 5 days after the date of submission.

You will able to select from 5 days prior to and up to 5 days after the date of submission for a COVID-19 note. 

You will only be able to select one single date for missing or having to cancel a flight on a Travel note. 

Your note will be delivered to you via email as a secure attachment. You will receive a password that grants you access to the note.

You can expect to receive the doctor’s note within an hour from the time of your submission.

If you have successfully completed and submitted our online request form, you will receive a confirmation message at the end of the process to state you will be receiving your note shortly. The doctor’s note will be sent to you as a secure attachment to your email within minutes of your submission.

No, you will not incur any charges if you were not approved to receive a note.

No, this service is not eligible for billing or reimbursement through insurance companies.

Once you have received a signed doctor’s note from us, you will not be able to receive any refunds. We run on a no-refund policy. If you need any changes or adjustments made to your note, we can accommodate your requests. Simply contact us at contact@mydrsnote.com or call us at 1-888-457-1844. 

Certainly, you have the option to print your note. However, please remember to retain the password you were provided so you can access and print your note as needed.
